APPLICATION FORMS
To begin the application process with the school, the following forms can be downloaded, completed, and submitted to the school office:
APPLICATION FORM	Submit this along with a $50.00 non-refundable application fee for each child seeking admission.
	You must also include a copy of each student's birth certificate.
	Copies of report cards, standardized test scores, and any other information relating to your child's academic experience should also be submitted.
PASTOR'S RECOMMENDATION FORM	Please give the form to your pastor to be completed and sent to us.
PARENT TESTIMONIES	Each parent should complete this.
REQUEST FOR STUDENT INFORMATION	Please give this form to your child's present/previous school to be completed and sent to us.
STUDENT APPLICATION FOR ADMISSION	Students in grades 6th-12th must complete this.
ASSOCIATION MEMBERSHIP STATEMENT	Please read and sign this statement.
When all of the above items have been received, an admissions interview with the Admissions Committee will be scheduled.
